National:

	http://www.humana-one.com/ - this link will take you to Humana's individual site.  From this link you can choose "Get a Quote".  Enter some basic information and your quote will appear on the next page.  This company offers list billing in Ohio, but some restrictions apply.
	http://www.assuranthealth.com/corp/ah/HealthPlans/GetAHealthInsuranceQuoteNow.htm - this link will take you to AssurantHealth's individual website to get a free online quote.  You can also save your quote and access it at a later time.  This carrier offers many individual options.  This company now offers list billing (effective 4/3/06).

http://www.aetna.com/members/individuals/health/quote.html - this link will take you to Aetna's individual website to get a quote on insurance.  This carrier allows you to apply online or get a quote and contact your local agent.
